Whichever you choose, contact the Ministry of Health and Long Term Care for assistance. You will be accessed by a group of care professionals to determine your need for care and type of services best for you.

The Community Care Access Centres in Toronto and York Region are responsible for arranging appropriate services such as physiology, personal care, home help service nursing care, social worker services etc. If it is so needed, you may be wait-listed for a nursing home as a long term planning.
